California Advancing and Innovating Medi-Cal (CalAIM) is a far-reaching, multiyear plan to transform California’s Medi-Cal program and to make it integrate more seamlessly with other social services. Led by California’s Department of Healthcare Services (DHCS), the goal of CalAIM is to improve outcomes for the millions of Californians covered by Medi-Cal, especially those with the most complex needs.

As a CalAIM Technical Assistance (TA) provider, we’re excited to be able to help transform the Medi-Cal system one successful client project at a time. We are consulting with safety net clinics and community-based organizations across California to stand-up and strengthen their Enhanced Care Management and Community Support programs.

The Providing Access and Transforming Health (PATH) TA Marketplace is a one-stop-shop website where entities can access free TA services from curated and approved Vendors like Hanserd Health Solutions.

Hanserd Health Solutions can offer you the guidance, tools, resources, and training you need to close the gap and succeed as CalAIM ECM and CS providers — and all costs for approved projects are paid by Department of Health Care Services.

Kaiser Permanente

CalAIM Medicare Medi-Cal Special Needs Plan Policy Implementation with Kaiser Permanente


In 2022, Hanserd Health Solutions served as a subcontractor supporting Kaiser Permanente’s policy implementation in the Northern and Southern California regions, taking on a program manager role and a subject matter expert in care management. Working with cross-functional leads and other subject matter experts, Hanserd Health planned the implementation of the new CalAIM Medicare Medi-Cal Special Needs Plan policy requirements. The team satisfied the requirements, problem solved barriers as they arose, and kept to a tight timeline.

Activities:

  • Facilitated core team meetings.
  • Provided technical assistance to the leads of eight workstreams.
  • Coordinated status reporting and steering committee updates.
  • Guided the development of training and communications materials.
  • Performed current-state assessment of care management offered to Medicare Medi-Cal members.

Alameda County

Whole Person Care Pilot


Hanserd Health Solutions served as a subcontractor supporting Alameda County’s Behavioral Health Care Services agency in the procurement, planning and design of the AC Care Connect Community Health Record (CHR) and Social Health Information Exchange (SHIE). Led by Hanserd Health, the Alameda County Care Connect CHR and SHIE were built, implemented, and continues to be operational. These electronic record systems have increased the opportunity for broad visibility and enhanced care coordination across physical, behavioral, housing, and social care entities.

Activities: 

  • Led the development of $16M RFP to procure technology partner.
  • Interviewed subject matter experts and developed use cases.
  • Facilitated a shared care plan design workshop.
  • Advised on care coordination practices, workflow design, voice of the customer and best practices around role definition, knowledge transfer and value proposition messaging.
